 I am the legal guardian of the player I am registering.

 I am the player and I am over the age of 18.

 MEDICAL CONSENT: I hereby state that the registered player is in good normal health and has my permission to participate in all activities. In addition, I authorize the camp staff to act for the player in the event of an injury or illness. I further agree that in the case of an accident involving the player while attending NVWBCA Showcase camp, I release the camp, sponsors, counselor and the Director from any and all liability.
